Create Fillable PDF Form

Add text fields, checkboxes, radio buttons, dropdowns, lists and signature fields to any PDF.

Free, no uploads, no sign-up. Runs 100% in your browser.

How Create Fillable PDF Form works

You draw fields on the rendered page. pdf-lib then writes standard AcroForm fields with appearances, so they work in Acrobat, browsers and phone apps.

Input: PDF. Output: PDF.

How to use Create Fillable PDF Form

  1. Drop a PDF.
  2. Pick a field type and click or drag on the page.
  3. Name the field and set defaults or choices.
  4. Save the fillable PDF.

Tips

  • Encrypted PDFs must be unlocked first with Unlock PDF.
  • Large scanned PDFs are mostly images. Compress PDF shrinks them the most.
  • Your original file is never modified; you always download a new copy.
